Only statistical information is desired in this section. No names should appear. In the first column list by title each rehabilitation employee individually. List in this column, also, rehabilitation agents who work under the supervision and control of the State board, but whose salaries are paid, in part or in whole, by other agencies, giving in the "Remarks" column the name of the cooperating agency and proportion of salary paid.

In the second column state the salary rate per annum, for rehabilitation work only, of each employee designated in the first column.

In the third column indicate whether the rehabilitation agent is a full-time worker; if a part-time worker, indicate proportion of time by fraction, such as 4 time, 1⁄2 time, o time, etc. It will be understood that a part-time worker is one who gives all of his time a part of the year, or part of his time all the year. Full-time workers entering or leaving the service within the fiscal year should be listed in the third column as full time.

SECTION II

Rehabilitated.-Cases to be reported in this section as rehabilitated must not only have been trained for employment, but have been placed in employment. 1. School training.-A program of training in which are present (1) organized instruction, (2) a responsible teacher, and (3) a definite plan of super

vision. By organized instruction is meant any plan of instruction embodying (a) a definitely determined and agreed upon content, and (b) a definitely determined order of presentation. Such training may be given in public, private, or corporation schools, by correspondence, by tutor, or by any other arrangement in industry which embodies all three of the characteristics given above.

2. Employment training.—If, in the program of training, one or more of the three characteristics of "school training" are absent, the case should be reported as having been rehabilitated through "employment training.”

3. Job restoration.-Rehabilitation through elimination of mental or physical factors which have prevented employment. In this form of rehabilitation no Vocational training is given.

SECTION III

1. Determined as eligible.-Cases having been investigated and determined as eligible for rehabilitation service.

2. In process of rehabilitation.-Cases in any stage between eligibility on one hand and training or employment status on the other hand.

5. Being followed up after placement.-In employment but still under observation.

INTERPRETATION AND USE OF ANNUAL STATISTICAL REPORT FORM

Section I.

The purpose of this section is to give information of a statistical nature regarding the rehabilitation staff. Information showing size of staff, salary rates, and time devoted to vocational rehabilitation service, is desired. In other words, the Federal board desires to ascertain through this section the size, character, and organization of the staff. (See "Instructions and definition of terms, p. 32.")

Section I.

Section II.

Item 1.-Attention is called to the fact that the term "school training" as used here, includes instruction by correspondence and by tutors.

Item 2.-Employment training in rehabilitation is fully defined in Bulletin No. 110, published by the Federal board. Only cases rehabilitated under conditions there set forth are to be reported under this item.

Item 3.-This item is to be used for reporting cases rehabilitated through placement alone. "Job restoration" covers placement at former or at new occupation with former or with new employer.

NOTE. The definitions of "school training" and "employment training" given on the report for Section II do not apply since the publication of Bulletin No. 110.

Section III.

This section calls for the live roll of cases on June 30 of the fiscal year for which the report is made. Items 1 to 5 are mutually exclusive.

Item 1.-Dependent upon practices in the States this item may or may not be used. It is provided for cases that have been determined as eligible, but have not been taken further because of lack of time or other contingency. Obviously, States that do not determine cases as eligible until actual rehabilitation is under way, will have nothing to report under the item.

Item 2.-Cases in any stage of rehabilitation between determination of eligibility on one hand and training or employment status on the other hand, are to be reported here.

Items 3 and 4.-Cases temporarily out of training because of sickness, recesses between school terms, and so on, should be considered as in training status.

Item 5.-For cases that have been placed, but are still under observation to determine fact of rehabilitation. This period of follow-up varies in the States. (See "Instructions and definitions of terms, p. 32,"
